Double the default tool round limit

Summary

• increase the default per-turn tool-round limit from 64 to 128
• update the limit regression to exercise all 128 allowed rounds and the rejected 129th call
• centralize the mock scenario threshold so future limit changes remain consistent

OpenAI Codex currently has no local tool-round cap; this keeps MicroCodex's safety bound while giving long-running turns twice the previous allowance.
