"Deceased Details Partially Available or Not Available" checkbox and reason text field missing from Death Declaration form

Bug description:
In the Death Declaration form, the "Deceased Details Partially Available or Not Available" checkbox is missing from the Deceased Person's Details section. This checkbox is expected to conditionally render a Reason text field upon selection, allowing the user to provide context for incomplete or unavailable deceased details. Currently, neither the checkbox nor the dependent Reason field are present in the form.

Steps to reproduce:

1. Login as any user who can create a death declaration
2. Start a death declaration
3. Navigate to the Deceased Person's Details section

Actual result:

• The "Deceased Details Partially Available or Not Available" checkbox is not present in the form, making it impossible to validate the conditional rendering of the Reason text field.

Expected result:

• The "Deceased Details Partially Available or Not Available" checkbox should be displayed at the top of the form.
• When the checkbox is checked, a Reason text field should conditionally appear below it.
