Adds a deterministic, local-only clinical-output use-policy gate for declared output categories, purposes, audiences, review states, decision-triggering status, and policy fingerprints. Denials and enforcement errors expose only stable reason codes, while safe serialization avoids echoing unknown declaration values. Adds synthetic offline tests and usage documentation.
