Normalize 2FA codes from prompts and
--auth-codeby removing matched bracketed-paste wrappers and whitespace, then require six ASCII digits before authentication. Preserve passwords unchanged.Report repeated 2FA challenges with a request for a fresh code instead of a generic network error. Add regression coverage for normalization, invalid codes, password preservation, and repeated challenges.
