Fixed the 'href' and related issues

Hi,

I've replaced 'href' with 'ui-sref', fixed some formatting, added some comments and some logging.

I tested the code by creating a starter app ($ ionic start myApp sidemenu) and then coping over the project files (replacing the contents of www).

index.html reference Ionic v1.0.0:
