docs: name the VS Code extension Whisper Code - #34

Settles the extension's identity in the integration plan: repo dimastatz/whisper-code, extension id whisper-code. The distinct name avoids confusion with the Wispr Flow dictation product; WhisperFlow stays the name of the server the extension speaks to. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
