- Move request-id, request-logger, logger, and audit into src/server/observability/
- Replace logInfo/logWarn/logError with a single log.{info,warn,error} surface
- Keep getRequestId and x-request-id behavior unchanged
- Update docs, server README, CHANGELOG, and server-patterns cursor rule
