[Formplayer] action-first media renderers with clear previews

Description

• Normalize media/attachment renderers (photo, audio, video, GPS, file) to an action-first flow: primary action at the top, then status/preview, then metadata.
• Surface secondary actions together (replace/retake/re-record and delete) near the primary area for faster corrections.
• Simplify previews and keep metadata readable; maintain QuestionShell layout for consistent title/description/error handling.
• No behavior changes to capture/record flows; this is UI/UX cohesion for media questions.
