What does this PR do ?

main PR: #2218

Fix hang caused by non-deterministic reduce-scatter operations.

Megatron-FSDP uses a set container to manage gradients, resulting in inconsistent gradient reduce ordering across DP ranks. This leads to different bucket execution orders during reduce-scatter, and when buckets have varying sizes, distributed operations will hang.

This issue has a low probability of occurring and is typically triggered only in certain model configurations. Setting TORCH_DISTRIBUTED_DEBUG=DETAIL can help identify instances of this problem.
