docs: sync tool counts to 357/75/9 + drop phantom protocols

- CLAUDE.md, plugin/README.md, server.json, specs/001-*/plan.md:
  align with actual registry — 357 tools, 75 groups, 60 Linux groups.
- plugin/README.md: drop fabricated protocols (ZeroMQ, NATS, MQTT,
  SNMP, NETCONF — never implemented). Real list: 9 (SSH, WinRM,
  PSRP, Telnet, K8s Exec, Serial, AWS SSM, Azure, GCP).
- src/mcp/tool_handlers/ssh_files_write.rs: replace prod
  unreachable!() with BridgeError::McpInvalidRequest for defense
  in depth if upstream validation drifts.
