Bug: database not initialized

Description

Following the Readme Quick start, app fails on missing tables.

Either one of the steps (make start-infra?) or the quick start should include uv run app database upgrade or something like this.

I encountered a similar problem, which may be due to changing the Docker volume mapping after using the latest pgsql18+ database. The path needs to be adjusted.

Could we pin PG to 17 for the time being?
