forgot password view of wp-login.php needs help

• Should have obvious link back to wp-login.php (without URL params) -- currently this has the label "Log In" and only shows up in the "Local" view (confused? yeah, it's a mess)
• IdP toggle button is very broken
• No SAML authentication CTA button

This is actually an improvement over the existing implementation!

Current version has two links below the lost password form, which would be better, but maybe with the way it floats at the bottom of the browser, it wouldn't really make sense... I think MVP is the way to go here, as this function shouldn't see much use with the way we want users to behave.
