Problematic document can break entire index

Describe the bug
When there is a document in the project that cannot be indexed, it can break the entire index - in particuar when the user tries to re-index the project.

Expected behavior
If there is a document which cannot be indexed, then only that document should be missing from the index and the rest should work.
