Revisit and refine plugin implementation

With the release of the latest alpha complete and Deja Vu on the horizon, now is the time to clean up, refactor, and refine the Visual Studio plugin implementation.

Architectural design decisions in place here are causing bugs. Small fixes and hacks have beaten them back, but it is probably time to seriously invest some effort into improving the architectural issues and fixing the threading bugs.
