Improved design for the TdiTableManager extensions:
TdiExternManager and Es2kExternManager are absorbed into TdiInfoManager and Es2kInfoManager, which are refinements of P4InfoManager.
TdiTableAnnex and Es2kTableAnnex are absorbed into TdiTableManager and Es2kTableManager.
TdiTargetFactory and Es2kTableFactory are absorbed into the implementations of PushForwardingPipelineConfig() in TdiTableManager and Es2kTableManager.
The resource type map is moved from P4InfoManager to TdiInfoManager.
