GRAPPA in 2D

I've added a 2D version of GRAPPA (i.e. handling acceleration in both phase encoding directions) which is essentially identical to the 1D version, but it has a different GRAPPA kernel - and I haven't tested full compatibility. It would be nice to have a universal 'do GRAPPA' function...
