Detect low-signal README and deprioritize them

The CLI currently includes the README, even when it contains no meaningful information. Add a deterministic check to identify low-signal READMEs (e.g. WIP, placeholders, one-line descriptions) and avoid treating them as primary explanation input, relying instead on file structure and extracted code signals
