Fix Password Reset Form Excessive Padding

Currently the Password Reset window looks something like this:

As clearly visible, there can be many possible improvements in this form:

1. Decreasing the size of input boxes (currently its height is excessive).
2. Eliminate Enormous Gap in the middle of form.
3. Optimize the Padding between its sub-components.
4. Add Placeholder text.
5. Add Divider to separate fields from Logout section.

Any other possible suggestions are welcome..!!
