Stoic SDK

I've refactored Stoic to allow both JVMTI-based injection (as has always worked) and building an SDK into an app, exported as a ContentProvider. This ContentProvider-based path works even in non-debuggable app - you can still dynamically inject plugins, but you can't call JVMTI-based APIs.
