How to achieve format on save etc using extension in VSCode. Tried several path options with biome.lsp.bin but just ended up confusing myself even more.

Why do you use biome.lsp.bin? Usually that's not needed.

However, if you need it, you have to make sure you provide the right value. The value you provided is incorrect. That binary is a small snippet that calls the real binary. The real binary changes based on architecture and OS.

All other extensions disabled and both not defined biome.lsp.bin or biome.lsp.bin: "./app/node_modules/@biomejs/cli-win32-x64" I get

Ok so "biome.lsp.bin": "./app/node_modules/@biomejs/cli-win32-x64/biome.exe" is the answer.
