Annotations Rating Field #17

Description
Add a ratings field to annotations.
This will be a 5 star field, with ability to reset to no rating. It should only be saved if a rating is provided.
Can probably be based around the Ext.us.Rating plugin, with a bit extra to make it fit into a form.
