- Launch configurations were no longer valid
- Fixed the code to launch rebar3, so that it injects the necessary code to register with the debugger
- Updated documentation
- In the process, made it easier to log from the extension
At the moment, the logger is only available in extensions.ts; we make it so that we can log from any module
This adds a default launch config to every new project, that's not how we want to handle this (and not working anyway)
At the moment it is not adding much value. The defaults it is using are out of date and it is not clear when they were needed. We can add again once we have a good motivation
