Improve ability to cross-reference and link to fields in docnode output

As per @martinpriestley:

field descriptions under a docnode / doctree haven't been given labels (or indeed headings), so we can't refer to them (except via the HTML)

Currently using definition lists. Look into whether doctree labels or target IDs can be bound to these.
