Collapse conditions/cancer types irrespective of capitalisation

Currently, if two conditions are the same except for capitalisation, the conditions are not collapsed.

E.g.
Colorectal adenocarcinoma
Colorectal Adenocarcinoma
Appear as two cancer types in the grouped record

This might be good to put in before SA Path as well. To discuss
