Support for MiniProfiler v4

This library will not run against MiniProfiler v4.

With MiniProfiler 4.0.165, get this run-time exception:
TypeLoadException: Could not load type 'StackExchange.Profiling.MiniProfiler' from assembly 'MiniProfiler, Version=4.0.0.0, Culture=neutral, PublicKeyToken=b44f9351044011a3'.

StackExchange.Profiling.MiniProfiler is now in the MiniProfiler.Shared assembly.
