Create an activity

Acceptance Criteria:
- fully styled and responsive
- encapsulates attendance creation with a form to fill in required and optional fields of an incident
- pulls and stores the list of active members in a local JSON file, updates whenever connected to wifi otherwise it uses stored version
- forward and back buttons to go between filling in fields and attendance
